Application Checklist:

SERVICE LICENSE RENEWAL

Renewals are due 30 days prior to expiration of license.

1. Application for Secure Transportation Service License
2. Certificate of Good Standing from the Colorado Secretary of State Office https://www.coloradosos.gov
3. Copy of the below written policies and procedures, if revised since first filing date. Email the office if no updates have been made since first filing.
   • General Policies and Procedures based on National Best Practices Guidelines, including staff background check policy
   • Operational and, if applicable, Medical Protocols
   • Training Procedures
   • Attestation that the Manager and Administrator meet the specific standards
   • Class A License: A policy which refers to physical restraint
   • Client Rights and related Policies and Procedures
   • Quality Management Program Policies and Procedures
4. Copy of Certificate of Insurance showing General and Professional Liability coverage, Worker’s Compensation coverage, and Vehicle Insurance coverage
   • Liability insurance for injuries in the amount of $1,000,000 for each individual claim
   • Liability insurance in the amount of $3,000,000 for all claims made against the secure transportation service or against its personnel from a company allowed to write liability insurance in Colorado
   • Minimum Vehicle Insurance Coverage as defined by §§ 10-4-609 and 42-7-103(2), C.R.S., with El Paso County stated as the certificate holder
   • Minimum Worker’s Compensation insurance based on Articles 40-47 of Title 8, C.R.S.
5. Secure Transportation Service Licensing – Staffing Requirements Form and proof of completion (for new employees not listed on first license)

VEHICLE PERMIT RENEWAL

Renewals are due 30 days prior to expiration of permit

1. Secure Transportation Vehicle Permit Application (need for each vehicle)
   • Proof of motor vehicle insurance
   • Photo of vehicle
   • Copy of registration
2. Secure Transportation Vehicle Evaluation Certification (each vehicle)
3. Secure Transportation Vehicle Inspection Report (each vehicle)
   • El Paso County completes this inspection
4. Quality Management Program Policies and Procedures, if revised since first filing date. Email the office if no updates have been made since first filing.
5. Fees
   • $250.00 for each renewal Secure Transportation License Application
   • $100.00 for each renewal Secure Transportation Vehicle Permit
